Hammer toe is a deformity that causes the toe to bend or curl downward instead of pointing forward. Although it most often affects the second or third toe, this deformity can affect any toe on the foot. Hammertoes and mallet toes are another common foot condition that usually results from poor foot care.

According to the Mayo Clinic, hammertoes and mallet toes are foot deformities caused by an imbalance in the muscles, tendons, or ligaments that hold the toe straight. A hammertoe has an abnormal curvature of the central joint of a toe. Mallet toe affects the joint closest to the toenail. Hammer toe and mallet toe usually occur on the second, third, and fourth toes.

Hammer toes and mallet toes can be caused by certain shoes, such as high heels or shoes that are too tight in the toe box, which can compress the toes and prevent them from lying flat. Hammertoes and mallet toes can also result from a crushing, entrapment, or fracture injury to a toe, increasing the risk of hammertoes or mallet toes developing on that toe. Another cause may be abnormal balance of the toe muscles. This imbalance leads to instability which can cause the toe to contract.

  • Wearing ill-fitting shoes
  • Trauma in which toes are trapped or broken
  • Damage to nerves and muscles from other conditions, such as arthritis
  • Structure or anatomy of the foot, such as an abnormally high arch, strained ligaments or tendons in the foot.
  • Pressure exerted by a bunion, that is to say when the big toe points towards the inside of the second toe.
  • Diabetic neuropathy. It can lead to abnormal biomechanics of the foot due to nerve and/or muscle damage.
  • Genetics or heredity

According to OrthoInfo, in the early stages of hammertoe, when the joint is still flexible, treatment usually consists of simple measures. This includes wearing shoes or boots with a toe box that is wide enough to provide minimal compression. Use orthotic insoles that help the toes flatten and spread and provide sufficient support for the metatarsal arch in the forefoot. If hammertoes are already formed, cushioned socks help protect the tops and tips of hammertoes and can reduce pain from friction and irritation.

If the affected toe is still flexible, changing shoes with a wider toe box and lower heels may allow the toe to realign and help reduce pressure and pain. If the toe has limited flexibility, a podiatrist or foot specialist may be able to straighten it simply by making an incision and releasing the tendon.
